Responsibilities:

  • Perform personal care needs after given a patient’s assignment and attending to his/her requests promptly. If unable to perform a  certain task, report to the Case Manager immediately.
  • Provide patient and family with positive communication techniques, with realm of ethical and respectful care, including confidentiality.
  • Meet safety needs of patient and use equipment safely and properly.
  • Give personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.

Qualifications:

  • Meets the training requirements of the State of California.

  • Understands hospice philosophy, comfortable providing specialized care to the terminally ill.

Qualifications:

  • Possesses a Certificate of Clinical Competence in speech from the American Speech and Hearing Association or meets the equivalent educational requirements and work necessary for such certificate and awaits opportunity for examination, or has completed necessary work and education requirements for certification and is in the process of accumulating the necessary supervised work experience.

Responsibilities

  • Directly responsible for the quality assured and cost effectiveness of services provided by assigned Patient Care Center hospice teams.  Responsible for managing all hospice service delivery with all direct reports of nursing, aide, social work, chaplain, volunteer and bereavement services.

  • Directs, implements and monitors standards of productivity, timeliness, and quality for all levels of clinical personnel and disciplines.

  • Provides the Patient Care Center Director with census, staffing, team productivity and other variance reporting as requested.  Identifies staffing patterns and needs of hospice.

Qualifications

  • License and current registration as a Registered Nurse.
  • Bachelor’s Degree in health related field with at least 5 years’ experience in hospice operations with progressive responsibilities and number of reports.
  • Evidence of progressive leadership responsibilities, including fiscal management is required.
