Responsibilities

  • Provide skilled nursing care to Agency patients:
    • Performs skilled assessments of patients including physical, psychosocial, and environmental factors.
    • Formulates a written Plan of Care and appropriate documentation is completed to admit patients to service.

  • Accepts and coordinates patient referrals and data, enters physician orders, securing necessary information needed according to Agency policies and procedures.

Clinical Supervisor-Home Health

Responsible for the day to day operations of an assigned team.  Plans, coordinates, manages and evaluates the activities of a multidisciplinary team to ensure the delivery of high quality home care services to patients consistent with company standards of practice, policies and procedures and applicable laws and regulations.
